This program is aimed at people aged 16 years and over and supports them to become more able to self-manage their own mental health and to increase their connections within their community.
PHaMs workers provide practical assistance to people with a mental illness to help them achieve their personal goals, develop better relationships with family and friends and to manage their everyday tasks. Support is offered on a one-to-one basis either in the person’s home or through office based appointments and also through links with community groups and activities.
